Test Eligibility:	I confirm that I am a Registered CFAA Trainee in good standing (ex: my membership is still valid and not expired). I confirm that I have successfully completed the CFAA Registration Theory Examination with a minimum passing grade of 80%. I understand that approximately 1,500 hours of practical, hands on experience in fire alarm system verification testing and reporting is recommended before attempting the Practical Test. This experience should include verification testing, troubleshooting, and completion of the required verification documentation. I have read and accept the test eligibility policy (*)

	I have not previously taken this test through HSMC or another CFAA approved training facility.
	I confirm that my first attempt was unsuccessful and that a minimum of one (1) month has passed since my first attempt. And I will provide proof of the date of my first attempt to HSMC prior to booking my test date.
	I confirm that my second attempt was unsuccessful, that the required minimum one (1) year waiting period has been completed, and that I have submitted the required CFAA Fire Alarm Trainee Log Book. And I will send the CFAA approval documentation to HSMC prior to booking my test date.

Refund Policy:	Cancellations received fewer than 10 business days prior to the registered start date will be subject to a $150 administration fee. Day-of cancellations and no-shows will be charged the full fee and are non-refundable. Qualified substitutions may be permitted at the discretion of Health and Safety Management College. Health and Safety Management College reserves the right to cancel, amend, or reschedule courses when necessary. I have read and accept the refund policy. (*)
Test Retake Policy:	To achieve a passing grade, you must score 80% or higher. If you score 79% or lower the following process applies: First attempt: You must wait a minimum of one (1) month before retaking the test. Second attempt: You must submit a completed CFAA Fire Alarm Trainee Log Book at the end of a minimum of one (1) year waiting period before you retake it. Each subsequent attempt will incur the full test fee. I have read and accept the Test Retake Policy. (*)
